Our practice provides free aid and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters and written information in other formats. We also provides free language assistance services. 

Services are provided in a nondiscriminatory manner. All health programs and activities provide:

• Reasonable modifications for individuals with disabilities, and appropriate auxiliary aids and services, including qualified interpreters for individuals with disabilities and information in alternate formats (large print, audio, accessible electronic formats, other formats), free of charge and in a timely manner, when such modifications, aids, and services are necessary to ensure accessibility, effective communications, and equal opportunity to participate to individuals with disabilities.
• Language assistance services, including electronic and written translated documents and oral interpretation, free of charge and in a timely manner when such services are a reasonable step to provide meaningful access to an individual with limited English proficiency.

If you believe that our practice has failed to provide these services or discriminated on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with the Director of Compliance by mail at 526 Main Street, Suite 302, Acton, Massachusetts  01720, by telephone  978-849-7582, by fax  978-371-0522, or by email: [email protected]. You must send the complaint within 60 days of when you found out about the issue.  If you need help filing a grievance, please contact the Director of Compliance.
